This Application form duly stamped, signed on each document of the Application form wherever
indicated be returned in original along with the form of Undertaking, Eligibility Criteria forms and
all other documents of the Application form documents duly filled in, Stamped and signed and with
all supporting documents enclosed. The format of declaration of rates shall be duly filled in and
enclosed. At the time of Submission of Application form these rates shall be available on the
eligible Applicant’s web site for all those who have a website. For all those who do not have a
web site, they shall immediately host one and ensure that this data is posted. Applications are
liable for rejection if this is not enclosed as per requirement or not made available on the
Applicants web site during the tenure of the Licence/Permission period within three months
from date of issue of the License/Permission.
